The practice Community Link Worker is Emma

You can be referred to the Link worker by the GP, Nurse or GP Admin Staff

Emma will focus on the priorities of our patients by listening to and understanding their non-medical problems such as:

Housing issues

Home modification or adaptions

Bereavement

counselling services

Referral on to money advice services

Domestic abuse

Coming to terms with a new diagnosis

And much much more

Emma is NOT able to help you apply for benefits/work capability assessment forms or appeal DWP sanctions however she can refer you onto services who are able to help you with this.

The Community Link Worker focuses on the priorities of those patients by listening to their non-health related concerns, understanding the issues that are at the top of their agenda and helping them access the most appropriate support.  Your first contact with the link worker will always be a telephone call to discuss your needs
